e911 Address Update

Currently when you update an e911 address using a template it will update the address but not allow you to enter a Customer Name.

After the template is applied it takes the name of the user and puts their name in the Customer Name.  We don't want that, we want to have the name of the business listed.

e911 template should have the ability to enter our Company Name in the 

Under Users-> Templates -> Create a new template.  Under User Details -> Emergency Address, there is no field for Customer Name.  When you apply the template to a user, it takes the user's name and places it in the Customer Name field on the phone.  We don't want the user name listed in that field, we want the name of the business.  

The end result desired: When calling 911, it shows the name of the business in the Caller Name field and the person's DID in the Caller ID field.
