We are new to RC. We would like each user to have two extensions - one for a physical phone and a "secret" one to be used in the smartphone app that only a few people know. Our boss likes the app but doesn't like all the calls ringing directly on it, he would like it if we could call him via the app when we have something important and all the other calls would route to his physical phone and/or voicemail.

You have a bit of a specialized situation here....one number cannot be assigned to multiple extensions.  You can create a 2nd extension for a user, but you would have to assign it a 2nd number, and then they can login to the mobile app or desktop app with the 2nd Ext. credentials.

You could have the User's Ext assigned 2 number on the same extension and then setup an Advanced Rule for the "called number"...and apply call handling rules to the two different numbers.

Would getting anot her number mean we needed to add more users? I.e. is there a cost associated with that?

not if you are not an AT&T customers.  Just add a virtual user: a New User without a phone.
