I just upgraded to Premium and names on incoming calls to the receptionist are showing up, but not when she forwards calls to people in the office. I was told that upgrading to Premium would give us this ability. How do we fix?

Keep in mind; if it's a BLIND transfer, the CLID & CNAM of the caller will be passed through.  SUPERVISED transfer does not, it shows the user transferring the call.

Just tested with Blind transfer and it worked.  That is a bit frustrating.  Now we are waiting for a headset to come in for our receptionist.  She was told she could transfer calls if she had a headset using the desktop app.  If she uses the desktop app right now, it goes to the person's computer and not their phone.  So is it safe to assume she would need the headset connected to the computer rather than her desk phone?

That is correct.  I, myself, use both and it works just fine.
