Voice Mails from Other Extensions Give Incorrect Identification

When "Extension A" leaves me a voice mail, the voice mail says I have a message from "Extension B". As far as a I know, I am the only one who has this problem with "Extension A" calls. However, another person has the same type of problem with "Extension C" and "Extension D", while no one else has that issue. It is odd. First, do we know what causes this issue? Second, do we know how to solve it?

08-09-2017:  It might be useful to take a look at both phones Users > Caller ID settings, and both phones "Nicknames" in the Users > Phones tab of the webportal.   You may find one of the phones has the inconsistency in one or both of those locations.

The phones are each only weird for one receiving extension. One of the calling extensions is the original user assigned the phone. The other was reassigned, but was never used by the extension it claims to be from. There are no inconsistencies in the names. Each of these phones is only misbehaving in identification with one other phone, but not the same phone.
