Salesforce integration click-to-call not working. Phone icon next to the phone number has a line thru it and nothing happens when clicked

Salesforce integration click-to-call is not working. Follow the instructions in the admin guide. The little phone icon next to the telephone number has a line through it and nothing happens when it is clicked. Instead there is a very cumbersome drop down menu that takes several clicks to reach the softphone. How do we activate the one click option?

Hello Kurt, 
It appears that your Call Center URL may still not be good. Please double check and Can you please call our technical support if it still does not work ?

The phone icon changes to one that is not strike through after a successful login

Successful login to which app? salesforce or RingCentral?

The menu to select a call is working ...

This screen indicates RingCentral for Salesforce app is installed but it does not tell if configured properly.  Can you please confirm if you have gone through the admin settings from Admin Guide

I've gone through the guide and paid extra attention to page 4. It seems fine.
We've inserted this url  https://intuilab--rcsfl.eu4.visual.force.com/apex/OpenCTIIndex999 as the CTI adapter url.

What else could be wrong?
