what could be causing this?

please help!! weird issue. calls come in and all phones ring. if one picks up it acts like it did but continues to ring on other phones and no one is on the call. this continues until everyone has answered. what could be the issue? we have dedicated ethernet to each phone all ports are open?? we are pulling our hair out trying to figure out this issue.
