I am not sure when the update was made as it is an infrequent task but I can no longer just reset a voicemail PIN.  It is now requiring I change all three items (password, PIN, and security question) before allowing me to hit save.  Is this intended behavior?

Save is blue after all 3 options entered

Yes, this is the intended behavior. When updating the voicemail PIN, the system now requires you to change the password, PIN, and security question simultaneously before you can save the changes.
