We don't text customers, we mainly receive texts, very minimum (i.e. 2 step verification type texts), do we still need to set a SMS campaign?

Outbound SMS is where this is focused. If you do not send texts and do not need to send texts, then no.
However, if you do send any texts, you will need to register.
More specifically, existing numbers will likely still be able to receive but will not be able to send. New numbers will not be able to do either.
