This can happen when the User has the app loaded on their mobile smartphone and they have changed the password via that.
The surest way to see what is going on is to review the Audit Log in Service Web.
I am fairly sure you will need Administrative access to do this. We have ours set this way.
If you do not have Administrative access you can request a log check from your RingCentral Admin at your organization.
