When I try to add an extension for testing in the sandbox environment, there is a $4.95 charge per extension. Since this is for testing only, that does not seem reasonable. How can I add an extension just to test a sandboxed app?

Simply go to users--> unassigned extension --> add users --> add user without device --> select without number:
Then you configure name and extension number in remaining process
