RingCentral Disabled Calling Number (outbound)

We keep getting a message from RingCentral saying "this number has been disabled. please contact your admin or ringcentral support" and I never disabled a number. I have no idea how this even got disabled.

Any clues on how/why RC disabled outbound calls to a number?
